Description
Responsibilities
- Coordinate discharge planning and continuum of care for hospitalized patients.
- Collaborate with physicians, nursing staff, social workers, and interdisciplinary teams.
- Assess patient needs and coordinate appropriate post-acute services and resources.
- Manage utilization review, care coordination, and discharge planning.
- Float to Adult Inpatient, ED, and Pediatric units as needed.
- Document patient care and case management activities in Epic EMR.
Qualifications
- Minimum 2 years of acute inpatient hospital case management experience within the last 5 years.
- Experience in Adult Medical/Surgical, Emergency Department, and/or Pediatrics required.
- BLS and ACLS certifications required.
- Epic experience preferred, but not required.
- Current RN license (pending license accepted per facility guidelines).
