Description
Title: Travel Licensed Master Social Worker (LMSW) – Acute Care Case Management/Discharge Planning
Location: Orange, CA 92868
Duration: 13 weeks
Shift: Days (5×8), 8:00 AM – 4:30 PM, 40 hrs./week
Requirements:
- Master's Degree in Social Work (MSW) required
- Active California MSW license required (pending license not accepted)
- Minimum 2 years of acute care hospital case management/discharge planning experience required
- Travel experience preferred (first-time travelers may be considered)
- Epic EMR experience required
- Experience with HMOs, IPAs, Medicare, and Medicaid
- Strong discharge planning and care coordination experience
- Ability to float as needed
Description:
- Provide medical social work services for hospitalized patients in an acute care setting.
- Complete initial psychosocial assessments within 24 hours of admission.
- Develop and coordinate individualized discharge plans.
- Coordinate post-acute care services, including home care, rehabilitation, long-term care, and durable medical equipment (DME).
- Collaborate with physicians, nurses, utilization management, insurance case managers, and multidisciplinary teams.
- Provide crisis intervention, patient and family education, advocacy, and community resource referrals.
- Assist with advance directives, protective services referrals, and discharge planning for diverse patient populations.
- Maintain timely and accurate documentation in Epic.
