Medical Social Worker (MSW) – Mission Viejo/Orange, CA

Shift: Days, 5x8-Hour (8:00 AM – 4:30 PM)

Minimum Requirements

Master's Degree in Social Work (MSW)

Active California MSW License

Minimum 2 years of recent acute care hospital experience in Case Management and Discharge Planning

Epic EMR experience required

First-time travelers are welcome (manager approval required)

Must be willing to float as needed

Primary Responsibilities

Conduct psychosocial assessments and patient evaluations

Coordinate discharge planning and transitions of care

Collaborate with physicians, nurses, case managers, and multidisciplinary teams

Educate and advocate for patients and families

Arrange post-discharge services, DME, home health, rehabilitation, and long-term care placements

Coordinate community resources, insurance benefits, and prior authorizations

Document assessments, care plans, and patient progress in Epic

Preferred Experience

Acute Care Hospital Case Management

Discharge Planning

Biopsychosocial Assessments

Care Coordination

Crisis Intervention

Psychiatric Patients, including 5150/Involuntary Holds

Knowledge of Medicare, Medicaid, APS/CPS, Hospice, Advance Directives, and Community Resources

Experience with medical-surgical patient populations; ICU and Mother-Baby experience is a plus

This position is ideal for an experienced Acute Care Medical Social Worker with a strong background in hospital case management and discharge planning who is comfortable working in a fast-paced inpatient environment.
