Requirements
- Active California RN License
- 2+ years of recent Acute Care Case Manager experience performing Care Coordination (within the last 3 months)
- Travel Case Management experience required (First-time travelers are not accepted)
- Epic EMR experience (Hard Stop)
- Experience with:
- Care Coordination
- Discharge Planning
- Patient Assessment
- Family Meetings
- Treatment Plan Development
- DME, SNF, and Ambulance Transport Arrangements
- Ability to work a variable 5-day schedule, including weekends as needed
- Willing to float to the Laguna facility
