THEMA Health Services is seeking a Travel Physical Therapist for a Home Health assignment in Phoenix, Arizona. Join a compassionate team dedicated to providing high-quality, one-on-one care that helps adult and geriatric patients remain safe, independent, and comfortable in their own homes.

Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments in patients' homes.
- Develop individualized treatment plans focused on improving mobility, strength, balance, and independence.
- Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Complete timely documentation while maintaining quality standards.
