Physical Therapist (PT) – Home Health

Job Type: Contract

Duration: 13 weeks
Schedule: 8-Hour Day Shift | 40 Hours/Week Guaranteed

Position Summary

We are seeking a licensed Physical Therapist (PT) to provide high-quality home health rehabilitation services. In this role, you will evaluate patients, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, restore function, and enhance independence while collaborating with an interdisciplinary care team.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop, implement, and update individualized Plans of Care (POC).
• Provide therapeutic interventions for orthopedic, neurological, and neuromuscular conditions.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on therapeutic exercises, home safety, mobility techniques, and use of assistive devices.
• Recommend and train patients on walkers, wheelchairs, canes, braces, prosthetics, and other durable medical equipment (DME).
• Complete accurate and timely clinical documentation, progress notes, physician orders, and care plans.
• Collaborate with physicians, nurses, and the interdisciplinary team to ensure coordinated patient care.
• Follow infection prevention, safety, and regulatory guidelines during patient care.
• Travel to patient homes within the assigned coverage area.

Required Qualifications

• Active Physical Therapist (PT) license in the practicing state.
• Bachelor's degree in Physical Therapy from an APTA-accredited program.
• 1–2 years of experience as a Physical Therapist in acute care, rehabilitation, or a related clinical setting.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
