Physical Therapist (PT) – Home Health
Location: Orangeburg, SC
Pay Rate: $60–$65/hr
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Shift: 8 Hours
Guaranteed Hours: 40 Hours/Week
Contract Duration: 91 Weeks

Position Summary

A licensed Physical Therapist (PT) who provides physical therapy to patients of all age groups in their homes. The PT evaluates patients, develops and implements treatment plans, and collaborates with physicians and the interdisciplinary team to restore or maintain patient well-being.

Responsibilities

• Perform patient evaluations and develop individualized Plans of Care (POC).
• Provide physical therapy treatments based on patient needs and physician referrals.
• Document evaluations, progress notes, and clinical records.
• Educate patients and families on therapy techniques, transfers, and the safe use of assistive devices.
• Recommend appropriate rehabilitation equipment when needed.
• Collaborate with physicians and the interdisciplinary care team.
• Follow infection control and safety protocols.
• Perform other related duties as assigned.

Required Qualifications

• Current State Physical Therapist license.
• Bachelor's degree in Physical Therapy from an APTA-approved program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and insurance.
• 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting.
• Experience with Homecare Homebase (HCHB).

Preferred Qualifications

• Previous home care experience.
