Travel Physical Therapist (PT) – Lymphedema Rehab

We are seeking a Travel Physical Therapist for a contract opportunity in Rockford, Illinois. This role requires a PT with lymphedema experience, as the candidate must have this skill set to be considered.

This is a great fit for a PT who has experience treating lymphedema patients and enjoys providing focused, patient-centered care in a collaborative rehab environment.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Provide treatment for patients requiring lymphedema management.
• Develop individualized treatment plans based on patient needs and functional goals.
• Treat mobility, strength, balance, and functional limitations as appropriate.
• Educate patients on home programs, safety strategies, and long-term condition management.
• Collaborate with the rehab team and other healthcare professionals.
• Complete timely documentation and progress reporting.
