Location: University of Mississippi Medical Center (UMMC)
Agency: Medical Solutions
Shift: 8-hour Days | Monday–Friday, 8:00 AM–4:30 PM
Charting: Epic
Patient Load: 15–20 patients/day
Setting: Acute Inpatient | Level I Trauma | Academic Medical Center
Job Description
Seeking an experienced RN/LPN Case Manager to provide acute inpatient case management and discharge planning for a complex, high-acuity patient population. The Case Manager will assess patient needs, coordinate multidisciplinary care, develop and implement individualized treatment and discharge plans, evaluate progress, and support timely, cost-effective transitions of care.
The position requires strong acute inpatient case management and recent discharge planning experience, excellent computer skills, and proficiency with Epic charting. The Case Manager will work collaboratively with physicians, nursing, social workers, rehabilitation, pharmacy, and other members of the multidisciplinary team.
Key Responsibilities
- Perform comprehensive assessments and identify patient care and discharge needs.
- Coordinate inpatient care and collaborate with multidisciplinary teams.
- Develop and monitor individualized treatment and discharge plans.
- Facilitate safe and timely patient transitions and appropriate post-acute placement.
- Coordinate complex discharges involving rehabilitation, skilled nursing, hospice, and other community resources.
- Monitor patient progress and barriers to discharge.
- Communicate with patients, families, providers, and care-team members.
- Document assessments, care coordination, and discharge planning activities accurately in Epic.
- Manage approximately 15–20 patients/consults per day across inpatient units.
- Consult with respiratory therapy and pharmacy as needed for discharge planning.
- Support utilization of available post-acute resources and appropriate placement options.
Required Qualifications
- Active RN/LPN state license.
- BLS certification.
- Minimum 2 years of acute inpatient medical case management experience.
- At least 1 year of recent experience in a large, busy hospital ED (500+ beds) within the last 3 years.
- Recent acute-care discharge planning experience involving a broad range of medical diagnoses.
- Previous travel experience in a Level I Trauma facility within the last 3 years.
