Description
Care Manager — Acute Care
Schedule: Days, 5×8 hours, 8:00 AM–4:30 PM
Facility: Acute care hospital
Patient ratio: 1:20
Caseload: Up to 25
Weekend: Every other weekend
Holidays: Must work 3 of 5 — Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day
Call/float: No scheduled float within the unit, but travelers may be required to float between facilities within the state
RTO: Maximum 7 days
Required Experience
- 2+ years acute-care case management/discharge planning
- Acute medical Care Manager experience with:
- Care coordination
- Admission review
- Discharge planning
- Medicare guidelines
- InterQual criteria
- Epic experience required
- Travel experience required
- California license required by start date; pending license can be submitted
- Acute behavioral-health-only experience does not satisfy the acute-hospital requirement
