Description
Registered Nurse (RN) – Case Manager
Location: West Hollywood, California
Start Date: September 14, 2026
Duration: 13 Weeks with possible extension
Shift: Days | 5x8-Hour (8:00 AM – 5:00 PM) (No on-call, No floating)
Key Responsibilities
- Provide comprehensive acute care case management services.
- Coordinate patient care with physicians, nurses, social services, transportation, and other interdisciplinary teams.
- Develop and implement individualized plans of care.
- Perform comprehensive discharge planning and coordinate appropriate post-acute services.
- Conduct utilization review and assess medical necessity.
- Utilize InterQual and/or Milliman Guidelines for utilization review.
- Apply knowledge of CMS, DRG, ICD-10, HIPAA, The Joint Commission, and other healthcare regulations.
Requirements
- Active California RN License must be in hand at the time of submission.
- Minimum 3 years of acute care Case Manager experience required.
- Current BLS Certification required.
- Epic experience highly preferred.
- Experience with acute hospital case management and utilization review.
- Experience with care coordination and discharge planning.
- Knowledge of InterQual and/or Milliman Guidelines preferred.
Strong understanding of healthcare regulatory requirements, including CMS, HIPAA, DRG, ICD-10, and The Joint Commission.
