Travel Physical Therapist (PT) – Home Health
Lake Zurich, Illinois | Northern & Northwestern Chicago Suburbs
Setting: Home Health
Profession: Physical Therapist (PT)
Contract: 13 Weeks
Schedule: Monday–Friday | 8:00 AM–5:00 PM
On-Call: None
Bill Rate: $90/hour
Estimated Mileage: 200+ miles/week
Home Health PT Opportunity – Lake Zurich, IL
Ardor Health Solutions is seeking a Physical Therapist for a Home Health travel assignment based in Lake Zurich, Illinois, providing coverage throughout the northern and northwestern Chicago suburbs.
This is a Monday–Friday, 8–5 schedule with no on-call, providing one-on-one physical therapy to patients in their homes.

- Approximately 200+ miles/week
- One-on-one patient care
- Independent daily routing
Qualifications Required
- Graduate of an accredited Physical Therapy program
- Current Illinois PT license or ability to obtain prior to start
- Current BLS certification
- Previous Home Health experience preferred
- OASIS experience preferred
- Valid driver's license and reliable transportation
- Strong evaluation, documentation, and time-management skills
- Comfortable independently managing a Home Health territory
Coverage Area & Mileage
This is a larger Home Health territory covering portions of the northern and northwestern Chicago suburbs. Coverage can extend from areas such as Evanston and Highland Park west toward Marengo.
Travel is estimated at 200+ miles per week, and because of the size of the territory, daily drive times may be significant depending on census and patient locations.
Candidates should be comfortable with a larger geographic territory. Exact routing, typical daily mileage, and how patients are geographically clustered should be discussed during the interview.
What You'll Do
- Complete PT evaluations and reassessments in patients' homes
- Develop individualized plans of care
- Provide gait, balance, strengthening, transfer, and mobility training
- Treat orthopedic, neurological, post-surgical, and medically complex patients
- Provide fall-prevention and home-safety education
- Educate patients and caregivers
- Collaborate with the interdisciplinary Home Health team
- Complete timely clinical documentation
