Position Snapshot
- Department: Case Management
- Focus: Inpatient discharge planning — not Utilization Review
- Schedule: Monday–Friday, 8:00 AM–4:30 PM
- Shift: 7.5 paid hours + 1-hour lunch
- EMR: Allscripts
- Manager: Reiny
- Orientation: 2 days hospital/EMR orientation + 1–2 weeks with a preceptor
Required Qualifications
- BLS required
- Minimum 1 year inpatient case management experience — must have
- PRI & SCREEN certifications required; can be completed online through the School of Albany
- Ability to independently manage at least 20 patients
- Critical thinking, time management, positive attitude, and strong interdisciplinary communication
- Critical care experience preferred
- Knowledge of CarePort, MCG/Milliman, and/or InterQual preferred
- Some UR experience preferred, although the position itself is discharge planning
Patient Load & Setting
- Usually Med-Surg or Tele
- Floors generally have 33–40 patients
- Typical staffing: 1 CM + 1 Social Worker
- CM caseload: approximately 17–25 patients
- Pilot floors: 1 CM + 2 SWs, with approximately 12–17 patients
- Must be comfortable rounding with physicians and the interdisciplinary team
Main Responsibilities
Discharge planning for patients going to:
- Home
- Community settings
- Skilled Nursing Facilities (SNFs)
Additional responsibilities may include:
- Obtaining housing
- Shelter packets for homeless patients
- Hemodialysis packets
- KCI/Wound VAC referrals
- Making community/facility referrals
- Coordinating discharge needs with patients, physicians, SWs, and other disciplines
UR is a separate department and handles insurance authorizations and transportation. Physicians handle medication reconciliation and pharmacy orders.
Weekend/Holiday
- Rare/occasional weekend or holiday coverage
- Requires manager approval
- Weekend coverage may involve multiple units
Dress Code
- Business casual or
- Any-color scrubs except Royal Blue or Maroon
- Neutral colors preferred
- Lab coat provided
