Description

Registered Nurse (RN) – Home Health

Job Type: Contract
Schedule: 8-hour shifts | 40 hours/week guaranteed
Contract: 13 weeks
Setting: Home Health
Travel: Local travel to patient homes required
On-Call: Required as assigned

Job Summary

We are seeking an experienced Home Health RN to provide skilled nursing care to patients in their homes. The RN will assess patients, manage individualized Plans of Care, provide clinical treatments, educate patients and families, and coordinate care with the healthcare team.

Responsibilities

  • Perform comprehensive patient assessments and identify clinical, psychosocial, and environmental needs.
  • Develop, implement, and evaluate Plans of Care (POC).
  • Complete accurate OASIS documentation and maintain timely clinical records.
  • Provide skilled nursing care, including medication administration, wound care, IV care, venipuncture, and catheter/drain care.
  • Educate patients and families on medications, treatment plans, safety, diet, and self-care.
  • Monitor patient progress and communicate changes to physicians and the interdisciplinary team.
  • Supervise and provide clinical direction to LVNs/LPNs and HHAs.
  • Coordinate referrals and participate in interdisciplinary team meetings.
  • Perform on-call duties as assigned.
  • Follow infection-control, safety, confidentiality, and regulatory requirements.

Requirements

  • Current RN license
  • 2+ years of Home Health experience
  • HCHB (Homecare Homebase) experience – Required
  • OASIS experience – Required
  • Current CPR certification
  • Valid driver's license
  • Reliable transportation and auto insurance
  • ADN or Nursing Diploma from an accredited program

Preferred: BSN and CHPN certification.
