LTC LVN/LPN – Travel Assignment

• Start Date: 09/01/26
• Assignment Duration: Traveler's choice — 13 or 26 weeks
• Shift: 4×12 Variable
• Shift Preference: Traveler will be assigned either Days or Nights based on facility needs; no routine rotation between days/nights
• Ratios: 1:20
• Call: No
• Float: Yes, within scope to meet facility needs

Requirements

• Previous travel experience: Required
• AK RN/LPN/LVN license: Must be active and in hand
• Experience: Minimum 1 year
• LTC experience: Required
• First-time travelers: Not accepted
• BLS: Required

Schedule

• 4×12 Variable
• 3×12 Variable

Vehicle Provided

• Facility will arrange a vehicle for the traveler during the assignment
• Vehicle may be facility-owned or provided through a rental car company
• Available up to 72 hours before contract start
• Must be returned within 24 hours after contract end
• Only the traveler is authorized to drive the vehicle
• Traveler must provide:
  • Valid driver's license
  • Proof of insurance
• Personal auto insurance must include rental-car coverage for accidents/damages
• Vehicle use is limited to around town / within 30 miles of the facility
• No off-road use or travel outside the 30-mile radius
• Vehicle must be returned during time off and a new vehicle issued upon return
• Tire chains are available at an additional traveler cost
• Vehicle must be returned:
  • Clean
  • With a full tank of gas
