Requirements

Active California RN License
2+ years of recent acute care RN Case Manager experience
Recent Care Coordination experience within the last 3 months
Epic EMR required (hard stop)
Previous travel experience required (first-time travelers not accepted)
Trauma Level I or II experience preferred

Responsibilities

Patient assessment and care coordination
Discharge planning and treatment planning
Lead and participate in family meetings
Arrange DME, SNF placement, and ambulance transport independently
Coordinate with HMOs, IPAs, protective services, and community resources
Support patients with complex social and behavioral health needs
