Registered Nurse – Case Management
Recent experience: Minimum 2 years RN Case Management
Experience in LTAC or STACH
Experience with MCG
Strong discharge planning experience across acute-care settings
Strong IDT collaboration
CA RN license — must be attached to resume
BLS certification through American Heart Association — must be attached to submission packet
Case management responsibilities include care coordination, utilization review/management, discharge planning, resource utilization, patient advocacy, and continuity of care.
