Description
Schedule: Days, 5×8 hours, 8:00 AM–4:30 PM
Facility: Acute Care
Role: Acute Care Case Management / Discharge Planning
Ratio: 1:25 days; weekend ratio listed as 1:45 in description / 1:25 in unit details
Weekend: Every other weekend, based on department needs
Orientation: 40 hours
Charting: Epic
Patient population: Neonates, adolescents, adults, geriatrics
COVID vaccine: Not required
RTO: Maximum 7 days; all RTO must be submitted upfront
Holiday requirement: Must work 3 of 5: Thanksgiving, Christmas Eve, Christmas Day, NYE, NYD
Hard Requirements
A candidate should not be submitted unless they meet these core requirements:
- At least 2 years of acute-care hospital Case Management/Discharge Planning experience
- Hospital experience is mandatory; behavioral-health-only experience does not count
- Travel experience required
- Must have the required CA state license by start date
- Pending license may be accepted for submission, but it must be active before starting
- Compact license must be in hand at submission
- Must be capable of independently performing:
- Patient assessment
- Care coordination
- Discharge planning
- Utilization review
- DME arrangements
- SNF placement
- Ambulance transportation
- Experience with HMOs/IPAs and authorization processes
- Experience with family meetings and treatment-plan development
- Knowledge of complex social/discharge situations, including homelessness, mental-health crises, substance-use treatment, advance directives/POLST, protective services, adoption/foster care
- Computerized documentation experience
