Position: RN Case Manager
Shift: Days – 7:00 AM–3:30 PM
Schedule: 5 × 8-hour shifts
Guaranteed Hours: 40 hrs/week
EMR: Meditech Expanse – Highly Preferred
Rate: $90/hr

Responsibilities

• Manage healthcare needs and coordinate quality and cost-effective outcomes for patients across the continuum of care.
• Perform admission, concurrent, and discharge assessments.
• Coordinate and monitor services and resources using a multidisciplinary team approach.
• Evaluate appropriate level of care and resource utilization throughout hospitalization.
• Develop and coordinate transition and discharge plans.
• Arrange appropriate post-hospital care and community resources.
• Collaborate with physicians, patients, families, insurance organizations, and government agencies.
• Apply knowledge of Medicare, Medicaid, commercial payers, and reimbursement.

Requirements

• 3+ years clinical experience in acute care – Required
• 3+ years experience in Rehab, Home Health, or Long-Term Care may be considered
• Massachusetts RN License – Required
• BLS – Required
• RN Case Management experience – Required
• Ability to perform comprehensive admission and discharge risk assessments.
• CCM/ACM – Preferred
• Meditech Expanse – Highly Preferred
• Strong communication, collaboration, negotiation, organizational, and analytical skills.
• Must work five 8-hour shifts; 10/12-hour shifts are not available.
• Must reside more than 50 miles from the facility.
