• Provide skilled physical therapy services to residents in a SNF setting.

• Educate patients and caregivers on exercises, mobility, and safe transfers.
• Collaborate with nursing staff, physicians, and the interdisciplinary care team.

Requirements

• Active New Hampshire Physical Therapist license or eligibility to obtain.
• Recent PT experience preferred.
• SNF experience preferred.
• Current BLS certification.
• Strong communication, clinical reasoning, and patient care skills.
