Home Health PT Opportunity – Yuma, AZ
Ardor Health Solutions is seeking a Physical Therapist for a Home Health travel contract with Amedisys in Yuma, Arizona.
This is a great opportunity for a PT who enjoys the autonomy of Home Health and true 1:1 patient care, helping patients improve mobility, safety and independence in their own homes.
Position Highlights
- Home Health setting
- 1:1 patient care
- Adult and geriatric population
- Complete evaluations and develop individualized plans of care
- Strengthening and functional mobility
- Gait, balance and transfer training
- Fall prevention and home-safety education
- Patient and caregiver education
- Help patients regain independence in their home environment
- Coordinate care with nursing, physicians and other members of the interdisciplinary team
