RN – Home Health | Newton, MA
📍 Facility: Mass General Brigham Home Care
📅 Start: 10/18/2026
⏳ Duration: 13 Weeks
🕐 Shift: 8:00 AM – 4:30 PM
📌 Weekend Rotation: Every 6 weeks
🚫 On-Call: No
Requirements:
• 1+ year Certified Home Health Care experience required
• 1+ year Case Management experience required
• Experience must be specifically with a certified home health agency
• Hospice, private-duty, or standalone case management experience does not qualify
• Must be able to travel to assigned patient cases
• Experience with admissions, care plans, patient/family education & case management
