Home Health PT Opportunity – Richmond, VA
Ardor Health Solutions is seeking a Physical Therapist for a Home Health travel contract with CenterWell Home Health in Richmond, Virginia.
This position provides individualized therapy in patients' homes, with the PT independently evaluating patients, developing and updating plans of care, providing treatment and collaborating closely with physicians and the interdisciplinary Home Health team.
Position Highlights
- Home Health Physical Therapist
- Patients across varying age groups
- One-on-one patient care
- Complete clinical assessments and determine therapy needs
- Develop, implement and revise individualized Plans of Care
- Treat neuromuscular, neurological and orthopedic conditions
- Gait, balance and transfer training
- Strengthening and functional mobility
- Patient and family education
- Train patients on walkers, wheelchairs, canes, braces and other adaptive equipment
- Recommend and order appropriate rehabilitation equipment
- Adapt equipment to meet the patient's home environment and needs
- Complete progress notes, physician orders and required clinical documentation
- Collaborate with physicians and the interdisciplinary care team
