Requirements
- 2+ years of Acute Care Case Management/Discharge Planning experience
- Hospital experience required
- Seasoned travel nurse required – first-time travelers not accepted
- California RN license required by start date
- Strong experience with Care Coordination, Discharge Planning, and Utilization Review
- Experience working with HMOs/IPAs
- Ability to independently coordinate DME, SNF placement, and ambulance transportation
