Required Qualifications
- Valid LMSW - Licensed Master Social Worker license in California
- LMSW - Licensed Master Social Worker (2 years of experience)
- LMSW - Licensed Master Social Worker experience within the last year
- BLS issued by American Heart Association
- Experience in a Hospital setting
- 2 years of recent discharge planning experience
- Case Management experience
Role Summary
- Coordinates care management, utilization review, discharge planning, and resource use
- Supports patient assessment, care planning, implementation, and evaluation
- Advocates for patients and supports continuity of care and cost effectiveness
