Required Certifications
- Active California RN License
- BLS — AHA or ARC accepted
- Nursys verification uploaded within the last 2 weeks
Required Experience
- Minimum 1 year RN experience
- Must have Home Health Case Manager experience — this is specifically required
- Experience with Home Health
- Experience with care coordination
- Experience with discharge planning
- Experience with disease management
- Previous Charge experience required
- Strong experience managing a patient caseload and coordinating interdisciplinary care
Preferred
- Epic experience
- Experience working with geriatric patients
- Experience coordinating with PT, social services, transportation, and other interdisciplinary teams
