Description
Pride Health is actively hiring a Registered Nurse (RN) – Hospice Case Manager to join our client's Hospice Team.
Location: Huntingdon, TN 38344
Pay Rate: Up to $58.00/hour on W-2
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekly Hours: 40 hours
Patient Care Setting: Hospice
Dress Code: Black or Royal Blue Scrubs
Position Overview
- Pride Health is hiring a compassionate and experienced Registered Nurse (RN) – Hospice Case Manager to join our client's hospice care team in Huntingdon, TN. This position is ideal for a dedicated hospice professional who can provide high-quality patient care, manage a growing patient census, and collaborate effectively with the interdisciplinary team.
- The team is currently lean staffed due to an increase in census, creating an opportunity for an experienced Hospice RN Case Manager to make a meaningful impact on patient outcomes and team operations.
Key Responsibilities
- Provide comprehensive hospice nursing care to patients in their homes and other designated care settings.
- Manage an assigned patient caseload, conducting assessments, developing care plans, and monitoring patient progress.
- Coordinate with physicians, hospice aides, social workers, chaplains, and other interdisciplinary team members to ensure patient-centered care.
- Respond to patient and family needs, providing education, emotional support, and guidance throughout the hospice journey.
- Complete timely and accurate clinical documentation, including patient assessments, care plans, and visit records.
- Participate in on-call coverage, including weekends, as part of a shared rotation among all Case Managers.
- Adapt to changing patient needs and increased census while maintaining quality and continuity of care.
- Communicate effectively with patients, families, caregivers, and the healthcare team to ensure seamless care coordination.
Required Qualifications:
- Active Registered Nurse (RN) license.
- Previous experience as a Hospice Case Manager.
- Strong clinical assessment, care coordination, and patient management skills.
- Ability to work independently and collaboratively in a team environment.
- Willingness to participate in weekly and weekend on-call rotations.
