Role Overview

• Department: Inpatient Case Management
• Primary function: Discharge planning
• Patient population: Primarily Med-Surg and Telemetry patients
• Typical coverage: Inpatient units

• UR: Separate department; handles insurance authorization and transportation arrangements

Required Qualifications

• BLS — required
• Minimum 1 year of inpatient case management experience — required
• Knowledge/experience with:
  • Discharge planning
  • Referrals
  • Interdisciplinary collaboration
  • Care coordination
• Ability to independently manage a patient caseload of at least 20 patients
• Strong critical-thinking, time-management, communication, and organizational skills
• Ability to round with physicians and the interdisciplinary team
• Critical care experience preferred
• Familiarity with CarePort, MCG/Milliman, and/or InterQual preferred
• Some Utilization Review experience preferred

Certifications

• PRI and SCREEN certifications are required.
• These can reportedly be completed online through the School of Albany and generally take a couple of hours each.

Caseload & Staffing

• Typical floor census: 33–40 patients
• Standard staffing: 1 Case Manager + 1 Social Worker
• Typical CM caseload: 17–25 patients
• Pilot floors: 1 CM + 2 Social Workers
• Pilot-floor caseload: 12–17 patients
• Candidates should be comfortable managing 20+ patients.

Typical Responsibilities

The Case Manager will primarily coordinate safe and appropriate discharge planning, including:

• Discharge to home/community
• SNF placement and referrals
• Housing resources
• Shelter packets for patients experiencing homelessness
• Hemodialysis-related packets/referrals
• KCI/wound-vac referrals
• Other discharge-related resources and referrals
• Rounding with physicians and the interdisciplinary team
• Coordinating with patients and other members of the care team

Schedule

• Monday–Friday
• 8:00 AM–4:30 PM
• 7.5-hour workday with a 1-hour lunch
• Breaks follow facility policy
• Management supports employees taking the full lunch break
• Weekend coverage: Rare, with manager approval; may involve multiple units
• Holiday coverage: Rare
