Job Title: RN Case Manager - Hospice
Location: White River Junction, VT 05001

Shift: Full-Time | Days

Position Overview:
Seeking an experienced Hospice RN Case Manager to provide compassionate, patient-centered care to individuals with terminal or chronic illnesses.
Requirements:
- Active Vermont and New Hampshire RN licenses OR an unencumbered Compact RN license.
- Graduate of an accredited nursing program.
- Minimum 2 years of experience.
- BLS required.
