Job Summary

Visiting Nurse and Hospice for Vermont and New Hampshire is seeking a Full-Time Hospice RN Case Manager to provide compassionate hospice care to patients with terminal or chronic illness.

The RN Case Manager provides pain management and comfort care while coordinating services with physicians, patients, families/caregivers, and the interdisciplinary team.

Requirements

• Licensed Registered Nurse in Vermont and New Hampshire, OR
• Unencumbered Compact RN License
• Graduate of an accredited school of nursing

Preferred Experience

• Medical/Surgical nursing experience
• Oncology nursing experience
• Hospice care experience

Responsibilities

• Develop individualized care plans in collaboration with the physician, patient, and family/caregivers
• Periodically review and recertify patient care plans as appropriate
• Coordinate patient care through interdisciplinary communication and case conferences
• Perform ongoing assessments of clinical, functional, and emotional status
• Identify patient problems and monitor progress toward goals
• Communicate significant patient changes to physicians and VNH colleagues
• Document evaluations, patient responses, and progress toward goals
• Educate patients and caregivers on care techniques and end-of-life symptom management
• Provide pain management and comfort-focused care
