Case Manager – Healthcare

Schedule

10-hour shifts, 4x10 schedule, with no set day off. Every 6th weekend as needed. No major holidays. Must be willing to float as necessary according to unit needs.

Assignment Details

Guaranteed Hours: 40
Contract Weeks: 13

Requirements

• 3 years of experience in a related field
• Extensive knowledge of current medical/surgical patterns of practice and acute inpatient care
• Experience with methods for assessing physical and mental impairment
• Understanding of the characteristics of illnesses and expected treatments
• Knowledge of benefits, plan contracts, eligibility issues, and requirements
• Bachelor of Science in Nursing (BSN) required
• Preferred certifications: Case Management Certification (CCM) or related

Responsibilities

• Utilize a collaborative process to assess, plan, implement, monitor, and evaluate options/services required to meet patient health needs.
• Communicate effectively and efficiently to promote quality and cost-effective outcomes.
• Manage the length of stay (LOS) and promote the efficient utilization of resources.
• Determine medical necessity and appropriateness of admission using established criteria.
