Travel Physical Therapist – SNF/Rehab
Bloomington, IN
Ardor Health Solutions is seeking a Physical Therapist for a travel Skilled Nursing/Rehab contract in Bloomington, Indiana.
Position Overview
This position works primarily with an adult and geriatric rehabilitation population, helping patients regain strength, mobility and independence following hospitalization, surgery, illness or injury.
Responsibilities may include:
- PT evaluations and reassessments
- Development and progression of individualized plans of care
- Therapeutic exercise and strengthening
- Gait and balance training
- Transfer and functional mobility training
- Fall prevention and safety education
- Patient and caregiver education
- Collaboration with nursing, OT, SLP and the interdisciplinary rehab team
- Discharge planning and recommendations for the appropriate next level of care
