Requirements
- Active California RN license
- At least 2 years of Acute Care Case Management experience
- Level 1 Trauma exp
- Knowledge of discharge planning, utilization review, care coordination, and the continuum of care
Responsibilities
- Manage care coordination and discharge planning for an assigned inpatient caseload.
- Review admissions, level of care, length of stay, and appropriate use of services.
- Arrange follow-up care and resources for safe, timely transitions.
- Document plans and communicate with the interdisciplinary team and payers.
