Orientation Dates: Week of 10/12/2026; Week of 10/26/2026

 

• Shift: 40 hr/Days (5x8's)
  • Mon-Fri - 8am-430pm

  • 13-week contract

     

• Is RTO Accepted? What is the Max:
  • No RTO is ideal and will be the candidate chosen first
  • 5 days or less in RTO (Candidates with more than 5 days RTO total will not be accepted)
• On Call Requirement: NO
• Certs: BLS
• License: Active CA LVN License
  • If CA License is expired, it will need to be renewed
  • If applying for a CA License, please call the Senator's office to expedite the application
• Experience:
  • 2 years of LVN experience required
  • Case Management; Med Tele
  • Supported patients and families through the discharge process, ensuring caregivers were prepared and patients had the resources needed to safely return home.
  • Served as a liaison between patients, families, and the RN, providing additional clinical support through patient rounding and communication.
  • Helped identify and address discharge barriers to improve patient flow and support timely transitions of care.
• Previously termed candidates will NOT be accepted
• Candidates with previous bad performance will NOT be accepted
• Travel Experience preferred, however, not required
• GQR proven performer highly preferred
• Will accept local travelers
• COVID Vaccine Requirement: Not required

Duties:

• Each LVN will be covering some sort of a Med Tele unit
  • 4 Units - Each unit has 24 beds
  • Range 20-24 patients
• Assess patient and caregiver readiness for discharge.
• Round on patients and communicate needs or concerns to the RN.
• Reinforce discharge education and ensure patients/families have necessary resources.
• Identify barriers to discharge and collaborate with the care team to resolve them.
• Support safe, efficient patient movement through the hospital system.
