Required Qualifications

Valid California RN license
Associate of Science in Nursing / Associate Degree in Nursing
BLS issued by the American Heart Association
2 years of Acute Care Case Management experience
Acute Care Case Management experience within the last year
Experience in a Long Term Acute Care setting
Short Stay experience

Role Focus

Care coordination and utilization management
Assessment, care planning, implementation, and evaluation
Discharge planning and continuity of care
Patient advocacy and resource utilization
