Description
RN Case Manager / Care Coordinator – Florida
📍 Location: Tallahassee, Florida
🕒 Shifts:
- Days: 8:00 AM – 4:30 PM | 1 weekend every 4 weeks
- Evenings: 12:00 PM – 8:30 PM | Rotating weekends
- Alternative: 11:00 AM – 7:30 PM | Mon–Fri, rotating weekends as needed
Requirements
- 3+ years of recent acute-care Case Management experience, including experience within the last year.
- Candidates with 3+ years of current Med/Surg, Telemetry, Neuro, ICU, PCU, or ED experience may also be considered.
- Home Health or Insurance Case Managers may qualify with:
- 3 years of total acute-care experience
- At least 1 year of acute-care experience within the last 5 years
- ADN or Nursing Diploma required
- Current Florida RN or compact RN license required
- Compact license holders must obtain an active Florida RN license within 90 days of hire
- APRN license may be accepted if current and compliant
