Travel Physical Therapist (PT) – Home Health

We are seeking a Travel Physical Therapist for a Home Health contract in Winter Haven, Florida. This opportunity is a great fit for a PT who enjoys independent field-based care, one-on-one patient treatment, and helping patients improve mobility, strength, safety, and functional independence in their homes.

Responsibilities

• Perform physical therapy evaluations and reassessments in the home setting.
• Develop individualized plans of care based on patient needs and functional goals.
• Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions.
• Help patients improve strength, endurance, safety, and independence in their home environment.
• Educate patients and caregivers on home exercise programs, fall prevention, and equipment use.
• Collaborate with physicians, nurses, case managers, and interdisciplinary care teams.
• Complete accurate and timely home health documentation.

Requirements

• Florida PT license required.
• BLS required.
• Home Health experience preferred.
• OASIS experience preferred.
• Valid driver’s license and reliable transportation required.
• Strong communication and independent time-management skills preferred.
