RN Case Manager | Apple Valley, CA

Duration: 13 weeks
Shift: Days, 8a–4:30p | 5x8s

Requirements:

2+ years Acute Care Case Management/Discharge Planning experience
Hospital Case Management experience required
Behavioral Health experience alone does not qualify
Experienced traveler required - no first-time travelers
Active CA RN license
Experience with care coordination, discharge planning & utilization review
Knowledge of HMOs, IPAs, CMS, DRGs & Joint Commission standards
Must independently arrange DME, SNF placements & ambulance transport

Additional:

Patient ratio: 1:25 (weekends up to 1:45)
Every other weekend as needed
No floating
Acute care facility
Adults, geriatrics, adolescents & neonates
Business attire or scrubs
Maximum 7 days RTO
