Ardor Health Solutions is seeking a Physical Therapist for a travel contract at a Skilled Nursing Facility in Philadelphia, Pennsylvania.
Position Overview
This is a SNF-based PT opportunity working primarily with adult and geriatric patients requiring skilled rehabilitation following hospitalization, illness, injury or surgery.
The Physical Therapist will evaluate patients, develop individualized plans of care and provide treatment focused on improving strength, mobility, balance and functional independence with the goal of helping patients safely transition to their next level of care.
Responsibilities
- Perform PT evaluations and reassessments
- Develop individualized plans of care
- Provide therapeutic exercise and functional strengthening
- Gait and balance training
- Transfer and mobility training
- Fall prevention and safety education
- Improve strength, endurance and functional independence
- Assess assistive device and equipment needs
- Provide patient and caregiver education
- Collaborate with nursing, physicians and the interdisciplinary rehabilitation team
- Participate in discharge planning
- Complete timely clinical documentation
