<h2>REQUIRED EXPERIENCE</h2><ul><li>Complex Case Management RN experience required</li><li>Experience managing medically complex inpatients required</li><li>Experience with patients with complex medical conditions, socioeconomic challenges, and mental health comorbidities</li><li>Experience with comprehensive case management and patient-centered care coordination</li><li>Strong nursing assessment and care-plan development experience</li><li>Experience with patient/family education, self-care coaching, motivational assessment, and adherence to care plans</li><li>Experience with end-of-life planning preferred</li><li>Epic experience preferred</li><li><strong>Education:</strong> BSN in Nursing or related field preferred/required, <strong>OR ADN with 5 years of Case Management experience</strong></li></ul><h2>SCHEDULE</h2><ul><li>8-hour day shifts</li><li>8:00 AM–5:00 PM</li><li>Monday–Friday</li></ul><h2>REQUIRED CERTIFICATIONS</h2><ul><li>No certifications specifically required</li><li>BLS not required</li></ul><h2>LICENSE</h2><ul><li>Active RN license required at submission</li></ul>
