• BSN or related Bachelor’s degree preferred, OR ADN with 5 years of Case Management experience
• Active nursing license required for submission
• BLS not required

Position Overview

The Registered Nurse Complex Case Manager (CCM) provides case management services to medically complex inpatients, including patients with complex medical conditions and/or socioeconomic and mental health comorbidities.

Responsibilities include:

• Providing patient-centered care coordination and case management
• Developing and monitoring patient/family care plans
• Conducting nursing assessments
• Supporting patient and family self-care and independence
• Motivational assessment and coaching for care-plan adherence
• End-of-life planning
• Coordinating care with interdisciplinary teams
