Ardor Health Solutions is seeking a Travel Physical Therapist for a 13-week skilled nursing assignment with Powerback Rehabilitation in Unity, New Hampshire!

This is an excellent opportunity for a PT who enjoys helping patients regain mobility and independence while experiencing the natural beauty of New England.

Located in western New Hampshire near the Vermont border, Unity offers peaceful country living, beautiful mountain scenery and convenient access to charming New England towns, lakes and outdoor recreation.

About Powerback Rehabilitation

Powerback Rehabilitation provides rehabilitation services focused on helping patients recover from illness or injury, improve mobility and regain functional independence.

In a skilled nursing environment, physical therapists work with patients who may be recovering from surgery, managing neurological conditions or experiencing mobility limitations associated with aging and other medical conditions.

Position Overview

As a Physical Therapist in a skilled nursing facility, you'll evaluate and treat patients with orthopedic, neurological and other conditions affecting their strength, balance, mobility and independence.

You'll develop individualized treatment plans and collaborate with nursing and rehabilitation professionals to help patients achieve their functional goals.

Responsibilities

• Conduct comprehensive physical therapy evaluations
• Develop and implement individualized treatment plans
• Provide therapeutic exercise, gait training and balance activities
• Assist patients with transfers and functional mobility
• Address strength, endurance and mobility limitations
• Treat patients recovering from orthopedic and neurological conditions
• Implement fall prevention and safety strategies
• Educate patients and caregivers on mobility and home exercise programs
• Collaborate with nursing, OT, SLP and other rehabilitation professionals
• Participate in discharge planning and maintain accurate documentation

Qualifications

• Active New Hampshire Physical Therapist license or applicable PT Compact privilege
• Graduate of an accredited physical therapy program
• Skilled nursing or geriatric rehabilitation experience preferred
• Strong clinical assessment and communication skills
• Ability to work collaboratively with an interdisciplinary rehabilitation team
