💪 What You'll Do

• Evaluate and treat patients in a skilled nursing and rehabilitation environment.
• Develop individualized treatment plans focused on restoring mobility and functional independence.
• Provide therapeutic exercise, gait training, balance training, and transfer training.
• Work with patients recovering from hospitalization, surgery, illness, or injury.
• Help patients improve strength, endurance, safety, and overall functional mobility.
• Educate patients and caregivers on home exercise programs and safe mobility techniques.
• Collaborate with nursing staff and the interdisciplinary rehabilitation team.
• Assist with discharge planning and recommendations for continued care.
• Complete accurate and timely clinical documentation.

📋 Requirements

• Active North Carolina Physical Therapist license or eligibility to obtain.
• Previous SNF/LTC or post-acute rehabilitation experience preferred.
• Current BLS certification.
• Strong clinical reasoning, communication, and patient care skills.
• Ability to collaborate effectively with an interdisciplinary rehabilitation team.
