To be considered for this Acute Care Case Manager travel position covering Petaluma and Healdsburg, CA, candidates need:

Required experience and skills:

• 2 years of experience, per the job-specific qualifications.
• Acute care hospital experience as a Case Manager — hard-stop requirement.
• InterQual criteria experience — hard-stop requirement.
• Epic experience — hard-stop requirement.
• Previous travel experience required.
• Skills in admission criteria, care coordination, discharge planning, CMS requirements, and HIPAA guidelines.
• Familiarity with ICU and med-surg populations; ambulatory and ED experience are preferred.

Schedule and floating:

• Five 8-hour day shifts, 8 a.m.–4:30 p.m.
• Every other weekend required.
• Must float between Petaluma Valley Hospital and Healdsburg Hospital. The weekly split varies and may include all five days at one campus.
• Listed daytime caseload is approximately 15 patients.
• 40 hours of orientation listed.

Submission and compliance:

• Submit all time-off and scheduling requests upfront; more than 7 days requires additional approval.
• Must commit to 3 of 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, and New Year’s Day.
• May submit to only one Providence facility at a time.
• Disclose previous Providence employment or assignments, including locations and dates.
• Former staff may return after 3 months, subject to confirmation.
• Provide 2 references covering 1 year of work within the last 3 years; charge RNs/leads are accepted.
• Locals within 50 miles are accepted at a lower agency bill rate; confirm candidate pay.
