Facility Overview
- 20-bed licensed unit
- Short-term skilled care only
- Typical patient ratio: 1:6
- Common patient needs include post-surgical recovery and chronic conditions such as COPD, CHF, diabetes, and renal failure
Required Qualifications
- Valid RN license in Illinois
- Associate of Science in Nursing / Associate Degree in Nursing
- Skilled Nursing experience within the last year
- 1 year of Skilled Nursing experience
- ACLS issued by American Heart Association or American Red Cross
- BLS issued by American Heart Association or American Red Cross
- NIHSS
Additional Requirements
- ACLS
- PALS
- Color blind testing
- Unit-specific competency exam
- Medication/pharmacology competency exam
- Competency exam with minimum passing score of 80% unless the Angoff score is higher
- Core Part I, II, and III
- OSHA mandatory training
- Physical exam within 30 days of start
- N95 fit test within 12 months of start
- Tdap vaccination within 10 years of start or signed declination
- Negative TB screening or chest X-ray within 1 year of start
- Drug test within 30 days of start
- Flu vaccination or declination during flu season
- Measles, mumps, rubella, hepatitis B, and varicella immunity documentation or acceptable alternatives
- Background check and education verification

- Government-issued photo ID
- Emergency contact form
- Required facility documents and acknowledgements completed before start
