Medical Surgical LPN Travel/Contract Position

Location: Rochester, New York
Contract Length: 13 weeks
Shift: Night (7:00 PM – 7:00 AM), 3 shifts per week (36 hours/week)
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed


Job Summary

  • Provide nursing care for medical surgical patients including those with COPD exacerbation, infections, failure to thrive, and social issues.
  • Float within skillset as needed.
  • Patient ratio during day shifts is approximately 1:5-6.
  • Charting experience with EPIC required.

Required Qualifications

  • Valid LPN/LVN license in New York (NY is not a compact state).
  • Minimum of 1 year Long Term Care experience within the last year.
  • Acute Care experience within the last 2 years.
  • Medical Surgical experience.
  • Ability and willingness to float within skillset.

Clinical Skills & Responsibilities

  • Vital signs, pain assessment, intake/output monitoring, isolation precautions, physical restraints, specimen labeling.
  • Oxygen administration via nasal cannula, feeding tube management, telemetry monitoring, 12-lead EKG.
  • Medication administration: oral, IM, SQ, NG/G-tube, ear and eye drops.
  • IV therapy with pumps, blood glucose monitoring.
  • Basic wound care including simple dressings and JP/Hemovac drains.
  • Bathing and mouth care, Foley catheter insertion (male/female), clean catch urine collection.
  • Communicate status changes to RN/MD in medical surgical/telemetry care setting.

Scheduling & Policies

  • Shift hours: 7:00 PM to 7:00 AM.
  • Must submit time off/block and special scheduling requests upon application.
  • Client may cancel up to three shifts during the 13-week contract without penalty.

Uniform

  • Scrub colors: Any combination of navy blue and white.

Required Documentation & Compliance

  • Government-issued photo ID required.
  • 7-year criminal background check within 3 months of start (including SSN trace, national criminal, sex offender, education verification).
  • EPLS/GSA/SAM and HHS/OIG checks required within 3 months of start and run monthly.
  • Foreign equivalency education evaluation if applicable.
  • Education verification from primary source required (not hard copy degrees).
  • Physical exam within 1 year of start, updated annually.
  • Fit mask (N95) fitting within 1 year, updated annually.
  • Negative TB test or chest X-ray within 1 year of start.
  • Required immunizations or titers: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
  • Drug screen within 30 days of start.
  • Seasonal flu vaccination or signed declination during flu season.
  • Snellen eye exam within 1 year of start.
  • COVID vaccination copy preferred but not required.
  • References (2 within 3 years) required for agency direct hires.
  • Employment application and skills checklist required.
  • Work history verification of at least 2 past employments.
  • State-specific checks including nurse aide registry (if applicable), criminal background fingerprinting (if applicable), disciplinary action, sex offender registry, and Medicaid exclusion list checks as required by New York state regulations.

Additional Notes

  • Locals accepted with a pay adjustment.
  • Orientation safety manual acknowledgment required.
  • Primary source verification of state license and NURSYS verification required within 30 days of start.
  • Medication/pharmacology competency exams and color blindness testing required as per profession.
  • Photo for badge must be a clear, passport-style photo with a solid background.
