Job Overview
- Location: Salem, Oregon
- Employment Type: Travel/Contract
- Shift: Day shift, 3x12 hours
- Hours: 36 per week
- Contract Length: 13 weeks
Role Summary
- Evaluate patients and develop/follow treatment plans in an acute care setting.
Required Qualifications
- Valid Physical Therapist license in Oregon
- Bachelor of Science or Master of Science or Doctorate in Physical Therapy
Preferred Experience
- Acute care experience
Schedule Notes
- Weekend rotation is included
- At least 3 weekend shifts in a 4-week rotation
- Possible holiday shift depending on timing
Required Pre-Start Items
- Epic EMR familiarity is preferred
- Signed job description
- TB history screening form
- Badge photo
- Physical exam within 1 year of start
- Fit testing within the first week of start
- Tdap vaccine or declination
- Negative TB screening or chest X-ray
- Flu vaccination or declination
- Drug screening
- Measles, mumps, rubella, hepatitis B, and varicella immunity/documentation
- References
- Skills checklist
- Background check and required exclusion checks
