Job Overview

Location: Salem, Oregon
Employment Type: Travel/Contract
Shift: Day shift, 3x12 hours
Hours: 36 per week
Contract Length: 13 weeks

Role Summary

Evaluate patients and develop/follow treatment plans in an acute care setting.

Required Qualifications

Valid Physical Therapist license in Oregon
Bachelor of Science or Master of Science or Doctorate in Physical Therapy

Preferred Experience

Acute care experience

Schedule Notes

Weekend rotation is included
At least 3 weekend shifts in a 4-week rotation
Possible holiday shift depending on timing

Required Pre-Start Items

Epic EMR familiarity is preferred
Signed job description
TB history screening form
Badge photo
Physical exam within 1 year of start
Fit testing within the first week of start
Tdap vaccine or declination
Negative TB screening or chest X-ray
Flu vaccination or declination
Drug screening
Measles, mumps, rubella, hepatitis B, and varicella immunity/documentation
