Overview
- Contract role in Tuba City, Arizona
- Monday–Friday, 8am–5pm
- Orientation every 2 weeks
Required Qualifications
- Valid Physical Therapist license in Arizona
- Physical Therapist (3 years of experience)
- Physical Therapist experience within the last year
- BLS issued by American Heart Association
- Bachelor's degree in Physical Therapy
- Experience in a(n) Outpatient setting
Clinical Scope
- Provides physical therapy evaluation and treatment for inpatients and outpatients across the lifespan, from birth through geriatric
- Includes specialty services such as wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, assistive technology/adaptive equipment evaluation and treatment, and Durable Medical Equipment Prosthetic Orthotic Supply Program
- Includes management and implementation of the Physical Therapy program, quality improvement, risk management, safety, policies and procedures, Joint Commission preparation, staff education, reporting, staff meetings, and in-services
Additional Notes
- Housing application must be submitted 2 weeks prior to start; housing is available at additional cost
- COVID and flu exemptions may be accepted after declination form is submitted
