Role Overview
- Contract home health RN position in Mesa County, Colorado
- Full-time, Monday-Friday day schedule with on-call rotation
- In-person role providing skilled nursing care in patients’ homes
Required Qualifications
- Valid RN license in Colorado
- Home Health experience within the last year
- Home Health (1 year of experience)
- BLS
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License
Preferred
- Home health experience
Key Responsibilities
- Perform comprehensive patient assessments
- Develop and update individualized plans of care
- Provide skilled nursing care and appropriate interventions
- Monitor patient progress and report changes to the care team
- Educate patients and families on medications, treatments, and care plans
- Coordinate care with LPNs, therapists, caregivers, and providers
- Complete timely and accurate documentation
- Participate in on-call rotation
