Job Opportunity: Home Health & Hospice RN

Location: St Johnsbury, Vermont
Employment Type: Contract
Contract Length: 12 weeks
Shift: Days, Monday to Friday, with possible weekend shifts
Hours per Week: 36

Job Description

• Provide a full range of professional nursing services to clients of all ages, including admission, evaluation, and care plan development.
• Collaborate with a team of homecare professionals and paraprofessionals to deliver coordinated care.
• Serve clients in Caledonia and southern Essex counties.

Required Qualifications

• Valid RN license in Vermont
• Minimum of 1 year of home health nursing experience within the last year
• Hospice nursing experience
• Basic Life Support (BLS) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
